Has anyone found real relief during a CHS episode beyond hot showers? I have seen references to haloperidol, capsaicin cream, and lorazepam in medical papers. What has actually worked for people?

In the ER we have had good results with haloperidol for CHS episodes not responding to standard antiemetics. Capsaicin cream on the abdomen also has emerging evidence — it works through TRPV1 receptors which interact with the endocannabinoid system.

Capsaicin cream was a game changer for me during episodes before I fully quit. Applied to my abdomen it reduced nausea intensity within about 20 minutes. Stopping cannabis is the real solution but capsaicin helped in the acute phase.

Standard antiemetics like ondansetron typically do not work well for CHS — this is actually diagnostically useful since CHS vomiting is resistant to conventional nausea medications. Haloperidol and droperidol work through different mechanisms and are more effective.
