    In the ER we have had good results with haloperidol for CHS episodes not responding to standard antiemetics. Capsaicin cream on the abdomen also has emerging evidence — it works through TRPV1 receptors which interact with the endocannabinoid system.

    We see it constantly at our hospital. At least one suspected CHS case every couple of days. Higher-potency products — especially concentrates — seem to accelerate how quickly CHS develops in susceptible individuals.

    The worst CHS cases I see in the ER involve people using concentrates heavily — dabs, wax, shatter. The THC doses are enormous and the gut does not handle chronic exposure to those levels well.

    How are you feeling between uses? Any persistent morning nausea or stomach sensitivity? If this seems truly isolated, probably not CHS. But if it happens again or you notice GI issues, see a doctor and be honest about cannabis use.

    The cyclic pattern plus ER visits is very concerning. Talk to her doctor about the cannabis use even if she minimizes it. The episodic pattern plus cannabis history is often enough to suspect CHS and pursue that diagnostic pathway.

    I work in emergency medicine and see CHS patients regularly. A few years ago it was rare. Now it is not unusual to have multiple cases in a single shift. Higher-potency products are accelerating this.

    Cyclic vomiting with hot water relief is the hallmark presentation of CHS. The episodic nature combined with the hot bath or shower symptom are the two most specific signs distinguishing CHS from other gastrointestinal conditions.

    Everyone is different — some develop CHS after 1 year, others after 10. Genetics may play a role. But the morning nausea pattern specifically is worth mentioning to a doctor soon.
